Things to Remember

 

Contact Logs:

Example: 01-01-07   TC   I called Bob’s home to speak with him about his recent trip to the emergency room. Bob was taking a nap but staff told me he was feeling much better.

Example: 01-10-07   TC   I called Bob’s conservator to let her know an investigation was opened today about allegations of Neglect. She thanked me for calling but said she was already aware of it.

Things to Remember

 

What can you say?:

*When making your contacts with the individual, family or conservator you should only provide them with the type of incident, date it was alleged to occur and if the individual is safe.

*When notifying about an investigative summary you provide the type of incident, date it occurred and say if the allegations were substantiated or not.

*If asked to provider additional information, refer the individual to the provider agency with their number for an incident or refer them to the investigator with their number for an investigation or to Dennis Beard if the questions are about a summary at 615-532-6549.

 

****Never give a copy or forward an Investigative Summary to anyone. ****

 

Let the IMC know whenever an incident has occurred and you have not received an incident report.
